2: Section #1: Plan Type

Plan Type

According to present data (Data Warehouse, October, 2016) Millennium Middle School's year to date average is at 96.5% from August 1016 to present; Millennium Middle School's present total enrollment is at 1,385 students (698 boys/687 girls).

 

 

2: Section #2: Purpose of Plan/Projection

Purpose of Plan:

In alignment with the district's attendance campaign Millennium Middle School's initiative includes the collaboration of students, families, staff and community members to improve the attendance rates at Millennium. Supporting the improvement in attendance (and working towards the reduction of chronic abteeism) is the responsibility of all stakeholders including the administrative team, school social worker, school counseling department and other support staff. VIsible posters, flyers, and announcements including, but not limited to September (Attendance Awareness month) include student and staff acknowledgement through activities iinvolving signing of an attendance pledge and other engaging activities. To confirm student commitment to iimproved attendance “I Pledged” stickers are distributed and discussed. To confirm family support the importance of attendance is discussed at the Annual TItle I Open House, school advisory meetings, and other parent workshops. To confirm staff commitment to improve attendance, concerns are  discussed with the school social worker/school counselor as soon as concerns arise and as prevention measures attendance is an agenda topic (as needed) at parent conferences and quarterly data chats with administrative staff and support staff.

Unexcused Absences

School Social Worker, community liaison, and school counselors monitor absences and are communication with parents for assistance/concerns as expressed by student, family, teachers, etc.

a) Refer to outside services as needed
b) Moniter student academic, social, personal needs
c) Provide in-house services as needed (awareness of academic programs, personal awareness support)
d) Monitor y child study team / RtI referral established as needed
 

 

Excessive Excused Absences
(5 or more in a single month)

School Social Worker/community liaison make contact w/ parents/guardians with purpose of establishing communication  re: assistance/concerns as expressed by student, family, teachers.

a) Refer to outside services as needed

e) Home visits
f) Monitored by child study team / RtI referral established as needed

 

Tardiness

Tardiness is addressed in accordance to schoolwide expectations; monitoring and documentation is mandatory.
1st tardy - Warniing
2nd tardy - Warning notice - writing assignment
3rd tardy - Parent contact / detention
4th tardy - Referral
Team Leaders will initial each referral upon review

 

Excessive Early Sign Out

Parent contact made to address concern and monitored accordingly. Cases discussed with MTSS/Rti team accordingly.  Home visits will be made if necessary.

 

Pattern of Non-Attendance
