Purpose of Plan

  1. Describe where the school is currently, using quantitative data. Include other data if needed, cite the source of the data, (e.g. Data Warehouse)

  2. Describe where the school wants to be (use precise data points), be specific and simply state what you plan to do, using the SMART guidelines in writing your goal statement.

    • Specific
    • Measurable
    • Attainable/Achievable
    • Results-focused, and
    • Time-bound.

Strategies To Be Implemented

Describe each strategy and include benchmarks/data points per quarter.

2: Section #1: Plan Type

Plan Type

Collins will take a proactive approach to decreasing chronic and excessive absenteeism.  An attendance flow-chart has been developed so that teachers, the attendance manager, guidance counselor, social worker, and administrators understand their respective roles in ensuring student attendance.  Quarterly incentives for perfect student attendance will be given to students AND parents as a means of reinforcing our attendance goals.  With this incentive plan, we plan to reduce our overall chronic absenteeism to 5%.  At the close of each quarter, a report will be generated for students with 5 or more absences.  Those parents who have not been contacted already will be called at that time to prevent the trend from continuing into the next quarter.

2: Section #2: Purpose of Plan/Projection

Purpose of Plan:

Student attendance is directly correlated with student achievement and poor attendance disrupts the instructional flow in the classroom.
The 2015-2016 attendance data indicates that 7.5% of our students demonstrated chronic absenteeism and 0% demonstrated excessive absenteeism.  By implementing a positive approach to addressing school attendance, Collins Elementary will continue to have 0% excessive absenteeism and reduce chronic absenteeism to 5%.

2: Section #3: Strategies to be implemented

Unexcused Absences

Parents of students who reach 3 unexcused absences in a quarter will be contacted by the classroom teacher and reminded of the importance of regular attendance.  Those with 5 unexcused absences will receive a BTIP notification and a meeting will be requested.  A determination regarding the reasons for the absences will be made and an attendance agreement signed.  Parents who do not attend a BTIP meeting will be notified to reschedule.  Parents who fail to show up for a rescheduled appointment will receive a home visit from our school social worker to sign an attendance agreement.

Excessive Excused Absences
(5 or more in a single month)

Our school's guidance counselor will contact the parent of any students who are demonstrating excessive excused absences.  An intervention plan will be written with the parent in order to determine the reason for the excessive absences.  Parents will also be asked to provide doctor's notes for extended absences.  In partnership with Memorial Health Care Systems, we are working to continue a partnership whereby our families are provided with mobile health servides at the park across the street from the school.

Tardiness

The attendance clerk will identify students who are demonstrating excessive tardies.  The parent will be contacted by the attendance manager to provide reasons for excessive tardies and to be reminded of the school hours.  If the tardiness continues, the assistant principal will contact the parent directly. Continued patterns will result in a home visit by the school's social worker.

Excessive Early Sign Out

A parent meeting and attendance intervention will be set up to determine the reason for the early sign outs.  Parents will be counseled by the assistant principal regarding ways to decrease early dismissal.  The school's social worker will meet with parents who are experiencing challenges with transportation, child care, etc...

Pattern of Non-Attendance
(Combinaton of the 4 previews strategies)

BTIP meeting
Attendance Agreement
Social Worker visit

Chronic Absenteeism

Each week, through BTIP, the school's attendance records will be monitored by the attendance manager and guidance counselor for early warning indicators that a student is developing a pattern of non-attendance.  Parent contact will be made and an intervention plan established.  The school's response to intervention team will be notified and a meeting will be held to address individual student concerns and to monitor student achievement correlations.  Chronic absenteeism will decrease by 1.5% as a result of the early interventions.
