Across tiers, data used to identify "gap" between expected and current outcomes

Why is it occuring?

Teachers and support staff are not utilizing school district resources with fidelity as well as documenting interventions or referrals as directed.

What are we going to do about it?

By June 2019, Dave Thomas Education Center and Charles Drew Family Resource Center will be optimizing in documenting and utilization of available resources to improve in Data-Based Problem Solving

 

Action Plan

  • Teachers use data chat forms with fidelity
  • Ensure effective use of available resources through instructional coaching (i.e. modeling, training by district and school-based experts)
  • Integrate and monitor student IEPS appropriately and effectively
  • Document Guidance interaction with middle and high school students through logs, TERMS, Credit Slips, graduation lists. 
  •  Monitoring DWH reports.
 
 

How will we monitor and measure our success?

  • Administration will monitor the graduation reports, data chat logs, and credit slips.
  • ESE Specialist will monitor and ensure IEPs are being followed with fidelity.
  • Guidance Counselors will input graduating students on log and input credits as teachers turn in.

Tier 1 academic practices clearly identify learning standards

Why is it occuring?

Instructional staff need to improve the MTSS/RTI process and procedure.

What are we going to do about it?

Increase school-wide documentation and maximize on all available resources school-based as well as within the district to improve MTSS/RTI process and procedure. Training by district and school personnel will occur to strenghten knowledge and application for instructional staff.

 

Action Plan

  • Improve MTSS team’s knowledge base on the Tier Levels
  • Reintroduce and monitor the implementation of the Tier levels to staff.
  • Increase documentation in BASIS
  • Monitor IEPs
  • Ensure behavioral contracts are followed and monitored.
  • Monitor data chat forms are utilized with fidelity

How will we monitor and measure our success?

  • Social Worker and Behavioral Specialist will monitor BASIS for TIER interventions and referrals.
  • ESE Specialist will monitor IEPs.
  • Administration will ensure MTSS/RTI team are well versed in RTI and TIER interventions.
  • Administration will also monitor data chat forms for instructional fidelity.
