heartbreaker to Palm
Beach Gardens. However,
they finished the
season with not only a
District Championship,
but also a great game in
which they knocked off
last years' state

Information for student-athletes and
parents regarding paperwork to begin athletic eligibility:
Student-athletes must turn in the following paperwork, as part
of their eligibility.
If any of the following items are incomplete or missing, the
student-athlete will not be able to participate in any way until
all items
are complete and approved by the Athletic Director.
This includes regular season practice and play, off-season
conditioning, fall leagues, summer camps and conditioning,
cheerleading camps, etc...
Please
submit the following items:
(a). A completed and doctor-approved physical
evaluation taken on June 1st, 2009 or after
**Please be sure to fill out all of your child's medical
history, personal information and current medications, so that
we are able to help as best as possible in the event of an
emergency
(b). A signed FHSAA Consent and Release from Liability
Certificate
(c). Proof of primary health insurance
**A copy of the student-athletes health insurance card will
suffice
***If you do not have primary health insurance, then you
may purchase a package through the Broward County Athletic
Association
(BCAA).

(d). An original birth certificate
**A photocopy of the original will be made and the birth
certificate will then be returned to the student-athlete
