Florida
Statute requires all Students
must receive the proper
immunizations before enrolling into a Broward County Public school.
The following
is a guide to the required immunizations. Students may be exempt for
religious or medical reasons with the proper documentation.

Vaccines

DTP: Diphtheria Tetanus Pertussis
DTP or DTaP
|
5 doses
If the 4th dose was given on or after the 4th
birthday, only 4 doses are required.
If 5 doses are given the 5th doses must be
given between the ages of 4 and 6 years.

|
IPV: Polio or OPV Oral Poliovirus
|
3, 4, or 5 doses
If the fourth dose of vaccine is administered prior to the
fourth birthday, a fifth dose of polio vaccine is required
for entry into kindergarten
|
|
Hep
B: Hepatitis B
|
3 doses
An alternative 2 doses series for adolescents 11-15yrs is
available.
|
|
MMR: Measles, Mumps, Rubella
|
2 doses
If the 1st dose was given sooner than students first
birthday, it is considered invalid and another dose is
necessary, The second dose may be administered before age
4-6 yrs provided more than 4 weeks have lapsed since first
dose.

|
Varicella
|
2 doses
For students in kindergarten and grades one through three
1 dose
For students in grades four through nine
|
|
Tdap: Tetanus – Diphtheria - Pertussis
|
1 dose
Students entering, attending or transferring to the seventh
or eighth or ninth grade

Varicella vaccine is
NOT required if there is a history of varicella disease documented
by the Health Care Provider on the 680 card.
Forms:
A Blue or white Florida Certificate of Immunizations (680) is
required for enrollment.
The 680 card can be obtained from the Broward County Department of
Health or a Private Physician.
The White Florida Certificate of Immunizations
(680) is also available from providers using Florida SHOTS. These
are electronically signed and certified DH 680’s.
They are available to schools that
participate in Florida SHOTS.

Question: What are the requirements for
the Varicella vaccine?
2 doses of varicella vaccine
are required for Kindergarten students effective 2008-2009 school
year. Subsequent year thereafter the next highest grade will be
included in the requirement.
(e.g. 2009-2010 Kindergarten and 1st
grade, 2010-2011 Kindergarten 1st & 2nd
grades etc)
Question: My child will be entering
the 12th grade this fall and we will be moving to Broward
County. He attended Kindergarten and first grade in Ft Lauderdale.
Will my child have to have a physical examination?
Since your child previously attended a
Florida school, a new physical examination (school physical) is not
required. This ruling applies to students entering a Florida school
for the first time; they must present a health examination
certificate (original) that was performed within 1 year prior to
school enrollment.
Question: Can my child attend school if
he/she has started the Hepatitis series but is not finished?
Yes, once a student has begun the
Hepatitis series he/she can attend school. The student must bring to
the school a blue 680 -B Florida certificate of immunization form.
The form will have an expiration date as to when the student is
required to return to his/her health care provider for the next
scheduled hepatitis vaccine.
Question: My family is moving to Broward
County from Iowa. My children are up to date on all their
immunizations. What is required of me so that my children can start
school as soon as possible?
Out of State immunization forms must be
transcribed onto a Blue Florida 680 form. This can be done by the
local Health Department or a Private Physician. You can enroll your
child in school however; you have 30 days to complete this process.
*Students may be admitted with a
temporary exemption for any of the vaccines. The vaccine must be
received prior to the expiration date for the exemption. If not the
student must be excluded from school until the vaccine is
administered.
*Students may have a permanent medical
exemption for one or more of the vaccines. The physician must state
the vaccines the exemption is for and the medical reason why it
cannot be given.
*Students may also be admitted with a
Religious Exemption that is issued by the Health Department. The
Broward county Health Department is the only entity that can issue a
Religious Exemption
