REASONS FOR CONTACTING PARENTS

• For questions regarding medication instructions

• Failure of the child to receive the medication for any reason

• Any error in the administration of the medication – contact immediately

• Any change in behavior or physical status, which might be attributed to the medication

• Changes in appearance of medication or expired medication

• Remind parent to bring in more medication.

• Parent not available to answer urgent question

• Any question not answered satisfactorily by the parent

• If signs of medication reaction become apparent, call healthcare provider and parent immediately

• Clarification of medication /treatment order written on authorization form
