Topic 3: The Matrix of Service Form

The hard copy of the Matrix of Services consists of four  to six pages.  Information about the student and the student's current eligibility is recorded on the first page.  Terms used on the first page are described on pages six and seven in the Matrix of Services Handbook.  The remaining pages contain checklists of services provided in each of the five domains and the special consideration checklist.

When you access the  Matrix on the EasyIEP, all the pre-populated information is filled in.  You will see:  Total Minutes in school per week, Minutes per week with non-ESE persons, and a list to check the names of the person (s) completing the Matrix that you will check.  You will then proceed to rate each domain.
