Contact the office of Professional Standards at (754)
321-0735 to report the allegation(s) of serious employee
misconduct.

Step 2 Submit
completed Personnel Investigation Request form and back-up
information to the Director of Professional Standards
via fax at (754) 321-0736.

| Step 3 Contact
the appropriate Deputy Superintendent, Associate Superintendent,
or Area Superintendent and advise of incident and facts
known to date. |
Step 4 Provide employee with written notice
of investigation when the investigation is authorized
by the Superintendent, identifying the nature of allegation.

| Step 5 Be advised
of the SIU Personnel Investigator assigned by Director
of Professional Standards to conduct personnel investigation.

Step 6 Keep the Office of Professional
Standards advised of any new information which surfaces
regarding the allegation(s).

| Step 7 Refer
the press/media to the Director of Professional Standards
or the Director of Community Relations. |
Step 8 Remember, F.S. 231.291, Personnel
Files, require confidentiality of any complaint and any
material relating to the investigation of a complaint
against an employee until the conclusion of the preliminary
investigation. |
