Students Requiring Medication During School:

  • Each year an "Authorization for Medication/Treatment" Form must be completed.
  • Dosage and administration must be clearly indicated on the form and signed by the student's healthcare provider and the parent/guardian.
  • Medication must be brought to school by the parent/guardian in the original container with the student's name, medication name and the dosage on the label.
  • A trained member of our staff will administer the medication to your child.
  • If the medication/dosage changes or is discontinued a new form must be completed.
  • For field trips that extend past school hours an additional form must be completed.
  • For medication/treatment of Diabetes a "Diabetes Medication/Treatment Authorization" Form and "Insulin Pump Medication/Treatment Authorization" Form must be completed.

No Shots, No School


Our students are precious to us. It is very important that we keep them safe from infectious diseases. Before entering a Broward County Public School, students must have the proper immunizations. Students may be exempted from the required immunizations for religious or other reasons - parents should contact the Health Education Services Department directly at 754-321-2272 for more information.


Minimum Required Immunizations:

  • Diphtheria Tetanus Pertussis (DTP) 5 doses/If 4th dose was given on or after the 4th birthday, 4 doses
  • Oral Polio Vaccine (OPV) 4 doses/ If 3rd dose was given on or after the 4th birthday, 3 doses
  • Measles, Mumps, Rubella (MMR) 2 doses


In middle school, ALL students must have the following shots in addition to those listed above:

  • Hepatitis B (series of 3 shots)
  • Varicella Vaccine (Chicken Pox)
  • Tetanus/Diphtheria (Td) booster ~ 7th grade only

"Parents and students, beginning with the 2011-2012 school year, the Code of Student Conduct Handbook will be distributed electronically. The Code of Student Conduct Handbook will be posted for review beginning July 1, 2011 on the Broward Schools Web site at www.browardschools.com.

"Beginning June 15, 2011, parents and students will be asked to enter email addresses in Virtual Counselor. The Code of Conduct Student Handbook will be emailed to parents and students beginning July 1, 2011."

No Child Left Behind (NCLB) federal legislation and Florida statue require that schools provide parents with an annual school status report at the beginning of each school year.
