What elements will be addressed?

Across tiers, data used to identify "gap" between expected and current outcomes

Why is it occuring?

Students are not meeting the expectations because their plans are not differentiated appropriately to meet their needs.

What are we going to do about it?

The MTSS team will consistently engage in efforts to facilitate Data-Based Problem Solving implementation and the team will consistently use data on implementation fidelity and for the purpose of school improvement.

Action Plan

1.  Professional Development will be scheduled to inservice teachers of how to appropriately differentiate individual needs of students.
2.  Teachers will develop instructional plans based on assessments
3.  The MTSS team will closely monitor the interventions academically and behaviorally at weekly meetings.
4.  CPS tem will meet with teachers to specify implementation of the plan
 

How will we monitor and measure our success?

Progress monitoring through the use of Running Records, formative assessments, performance-based assessments.  Administrators and coach(es) walkthroughs will be used as an additional progress monitoring tool.  Records will also be reviewed on BASIS.

Effective data tools are used appropriately and independently by staff

Why is it occuring?

Teachers are not utilizing the data tools effectively due to the need for more training on the use of the tools.

What are we going to do about it?

The leadership team will ensure the availability of tools and necessary training to effectively use the data tools for the purpose of continuous school improvement.

Action Plan

1.  MTSS will monitor and provide assistance as needed for teachers on using data tools at weekly meetings.
2.  Professional Development will be provided to train teachers on the use of the tools adn managing data to improve student instruction.

Classroom Observations
Records Review (BASIS)
Collect tvidence to support implementation (MTSS/RTI) meeting dates and professional learning sessions
