Data are used to identify reasons why students are not meeting expectations

Meet to discuss the individual student's needs and develop and plan.

A plan will be developed to target a specific behavior or academic need. 

The team members will meet periodically to discuss the goal and achievements made. If the goal needs to be altered, the team will adjust as needed. 

Teachers will use resources provided to track and measure the progress of students. This data will then be discussed after 6 weeks of implementation with the team.
