School Name Plantation Park ES (1251) School Year 2018 - 2019 Resources for implementation of MTSS are addressed through data-based problem-solving Students have gaps in their learning and there is a need to provide targeted academic support. With targeted academic and/or social/emotional interventions our students will be closer to proficiency. We are providing coaching and training to teachers/support staff. We are also ensuring that interventions are aligned to individual student needs. 1. Ensure correct identification of students of students in need of academic support and monitor interventions 1. Monitor data to see progress Case Managers and CPST 2019-03-20 2019-05-22 Data sources are used to evaluate the fidelity and impact Data is available however it is not being evaluated as suggested to drive planning and instructional decisions. Teachers will bring intervention data sources to CPST and monthly data chats to evaluate the elements of MTSS across academic, behavioral, and Social/Emotional areas. 1.
