What elements will be addressed?

Data are used to identify reasons why students are not meeting expectations

Why is it occuring?

Students individual plans are not meeting expectations due to the lack of fidelity of implementation for all Tiers of instruction.

What are we going to do about it?

We are closely monitoring the students Tier One and ensuring that it is being implemented with fidelity and that additional Tiers are prescribed.  The plans address specific needs and areas for growth.

Action Plan

Quarterly review of students in the MTSS process through Grade level data chats, individual teacher chats and CPST meetings. All teacher Tier One strategies will be reviewed in BASIS and progress on all levels of Tier instruction will be monitored through the SWAG folder.

How will we monitor and measure our success?

 On a quarterly basis, we will document the rate of success for our students in Tier 3 MTSS and monitor their level of progression.

What elements will be addressed?

Tier 3 academic practices are based on students' needs, aligned with Tier 1 and Tier 2

Why is it occuring?

Assessments that link directly to skills taught have not been created/utilized with fidelity.

What are we going to do about it?

Assessments will be created prior to thye remediaton process to determine effectiveness of interventions.  Grade level data points/charts will include the remediation cycle.  

Action Plan

  • Professional Learning Communities will include the C.A.R.E. (Curriculum/Assessment/Remediation/Enrichment) process will a focus on the remediation process.
  • School-wide RtI block will be established to address Tier II and Tier III needs of students.
  • The Leadership Team will collaborate with teachers to plan RtI lessons that meet the needs of students.
  • Instructional Focus Calendars will include the remediation cycle.
  • Grade level data will be posted at the completion of the C.A.R.E. cycle .

How will we monitor and measure our success?

  • Success will be measured by the Self Assessment of Muti-Tiered Systems of Support (SAM) domain average increase
  • Data review sessions will determine effectiveness of interventions.
  • Professional Learning Communities will include data comparisons which include the remediation cycle.
