What elements will be addressed?

Student progress specific to academic or behavior goals are monitored

Why is it occuring?

The leadership team and instructional staff are not consistently engaging in the monitoring of student progress specific to academic, behavior and social-emotional goals specified in the individual student interverention plans. 

What are we going to do about it?

The leadership team and instructional staff will consistently engage in the monitoring of student progress specific to academic, behavior and social emotional goals specified in the individual student interveion plans through bi-weekly Professional Learning Commmunities and MTS/RTI meetings.

Action Plan

Bi-weekly professional learning communities faciliated by Leadership and Instructional staff will address how to find and collect the social, emotional and academic data of students (BASIS, teacher notes, mentor points sheets and notes, parent conference data, student feedback).

Instructional staff and the leadership team will assist students with taking the LEAPS baseline assessment and the weekly practice of foundational skills for social emotional development taught within the LEAPS lessons. 
Leadership teams and instructional staff will participate in weekly MTS/RTI meetings to consistently monitor specific student academic, social and emotional skills. 

How will we monitor and measure our success?

-PLC- Bi-weekly notes and teacher evidence of student academic and behavioral progress
A decrease in referral data in BASIS
-75% of students participation in both baseline testing and weekly practice of foundational skills for social emotional development by the end of May 2019.

Effective data tools are used appropriately and independently by staff

Why is it occuring?

The leadership team and instructional staff are not consistently and independently using available data tools that can track and graphically display academic, behavior and social emotional data. 

What are we going to do about it?

The leadership team and instructional staff will consistently use available data tools that can track and graphically display academic, behavior and social emotional data during bi-weekly Professional Learning Communities and MTS/RTI meetings. 

Action Plan

Bi-weekly Professional Learning Communities facilitated by Leadership and Instructional staff will address how to appropriately and independently use available data tools that track and graphically display academic, behavior and social emotional data of students (BASIS, virtual counselor, teacher notes, mentor point sheets and notes, parent conference data, LEAPS data, student feedback)
· Instructional Staff and the Leadership team will use LEAPS baseline assessment data and classroom lessons to track and display behavior and social emotional data
· Instructional Staff will assist students with viewing and tracking academic progress using available data tools (I.e., virtual counselor)
· Leadership team and staff will participate in weekly MTSS/RTI meetings to consistently use available data tools to track academic, social and emotional data
· Leadership team and staff will use data tools to share academic, social and emotional data gathered within the MTSS/RTI meeting with their designated Professional Learning Communities (bi-weekly)

· Teacher surveys
