2: Section #1: Plan Type

Plan Type

Attucks Middle is located in Hollywood, Fl 33020. Our school current enrollment is 640 students. We have aproximately 366 males and 274 females. Eighty-eight percent of the population is Title I, and the average daily attendance rate is 96.7%(Aug. - Jan 2016). This information was retrieved from data warehouse by our school social worker.

According to Behavior and Academic Support Information Systems(BASIS 3.0), Attucks Middle has a total of 21 chronically absent students, and 5 students with excessive absences.  Our goals in implementing these strategies is to reduce the number of students who are chronically absent by 2%.

 

 

2: Section #2: Purpose of Plan/Projection

Purpose of Plan:

The purpose of the attendance plan is to encourage families to comply with the State's mandatory school attendance policy and to decrease chronic and excessive adsenteeism by 1% in the 2016-2017 school year.

 

 

 

2: Section #3: Strategies to be implemented

Unexcused Absences

1.  Teachers are to take attendance every period daily within the first ten minutes of each class.
2.  Be sure you are familiar with all attendance codes.
3.  When a student is absent (unexcused) for 3 days, teachers must contact the parent.
4.  School counselor will run an attendance report bi-weekly, which will show students with 4 or more unexcused absences. Parents will be 
     contacted by counselor or parentlink. School counselor will also conference with student.
5.  Students who unexcused absences increase to 5 or more will be referred to social worker. 

 

Excessive Excused Absences
(5 or more in a single month)

Social worker is mobilized to determine the cause of the issue and provide support services to those students. A verification of illness letter is implemented to determine whether absences are legitimate.  Student will also be discussed by the CPS/RTI Team.

 

Tardiness

Students are referred to guidance, parents are contacted, conference is scheduled and plan of action is created to ensure compliance with school attendance policies.

 

Excessive Early Sign Out

Students are referred to guidance, parents are contacted, conference is scheduled and plan of action is created to ensure compliance with school attendance policies.

 

Pattern of Non-Attendance
(Combinaton of the 4 previews strategies)

Teachers call parents to notify, if that is unsuccessful, counselor will attempt to contact parent.  Based on the results of counselor parent contact, the student will be referred to social worker. When all else fails, the social worker will send a referral to CIN/FINS.

 

Chronic Absenteeism

Chronic Absenteeism is handled using the above strategies, and referred to the social worker.  These students are also discuss at RTI meetings.
