Print the Transcript Request Form or follow the instructions listed below (electronic requests are NOT accepted). Your request must include the following information:

1.      Former student's name used in school (include married name, if applicable)

2.      Last school attended in Broward County Public Schools

3.      Last year attended

4.      Date of birth

5.      Social security number (Optional)

6.      Address where records are to be sent

7.      Daytime Telephone Number

8.      Signature of the Individual to whom the records belong

9.      COPY OF PICTURE ID

10.    Fee *see form- $2.00/official or $7.00/certified each (cash or money order only) Do not send coins
 

Processing time is approximately 7-10 working days.

IF YOU GRADUATED OR LAST ATTENDED AFTER 2006 SEND YOUR REQUEST DIRECTLY TO YOUR HIGH SCHOOL.
