Participant Accountability Guidelines
Participation in the Digital Divide Program is easy--download the application that applies to your site type (school or district program), complete, submit by CAB e-mail to "Digital Divide", then wait for a decision. After a favorable decision is received, what comes next?

Determine the number of families interested in participating in your training. Following, notify the Program via e-mail (reference the Remedy ticket number in your heading and address to Remedy, cc: Digital Divide) on the type and quantity of computers desired.

Decide on a training date. Provide the training date information to the Program via e-mail (reference the Remedy ticket number in your heading and address to Remedy, cc: Digital Divide).

Download all applicable Digital Divide Forms:
Equipment Release Form (ERF)
Forms Checklist

Complete your training. Make sure you have a completed, signed ERF and SEUSLA for each participating family before families take the Digital Divide computers home.

Designated contact person for the site completes and signs the Forms Checklist and returns it to Digital Divide along with all completed Digital Divide ORIGINAL forms and Digital Divide Training attendance sheet.

If you have equipment left after the training, please contact Digital Divide via CAB e-mail (reference the Remedy ticket number in your heading and address to Remedy, cc: Digital Divide) so the Program may proceed to schedule a pick-up date for remaining equipment.

Upon receipt of all Required forms, and pick-up of any Digital Divide equipment (if any), Digital Divide will proceed to RESOLVE the Remedy ticket. Site will be notified by Remedy that the ticket is now RESOLVED (CLOSED).
